Attestation of Power of Attorney to be executed in India. PoA/Affidavit (s) can be executed either on a non-judicial paper or stamp paper as per the applicant’s requirement.
How to apply:
i) Fill up the Miscellaneous form online.
ii) Take Print-out of the online filled in application form. The form should be duly signed,  photographs and necessary documents (please visit check list)
iii) Book an appointment (link)
iv) Book prepaid airway bill with POST/Courier service in the Netherlands for receiving processed documents. This should be submitted along with an A4 size self-addressed envelope
v) Submit application for Passport alongwith necessary documents and prepaid A4 size self-addressed envelope at Embassy of India in The Hague (Buitenrustweg 2, 2517KD, The Hague.

Documents required

LIST OF DOCUMENTS REQUIRED

1. A Miscellaneous form duly filled in and signed
2. One photograph (5 cm * 5 cm) with white background and clear and complete face with shoulders visible.
3. Original Indian passport and copy of the same (first and last page) or Original OCI/PIO card and copy of the same (first and last page)
4. Original Dutch residence permit card and copy of the same (front and back)
5. An additional copy of the Power of Attorney document.
6. Please be informed that there is no provision for making photocopies at the Embassy.

ADDITIONAL DOCUMENTS AS PER YOUR CASE
In case of applying for Power of Attorney, the applicant has to affix signature on each page of the document (before Embassy staff at the counter) and the witnesses to sign (before Embassy staff at the counter).

Passport and residence permit card (if applicable) of two witnesses and copy of the same (front and back).

Witnesses must be present at the time of application submission.

For OCI/PIO card holders, applicant may follow the same procedure for Indian nationals and must provide a recent Uittreksel with address apostilled by the district court (Rechtbank)

In case of applicants of other nationalities, they may first get the PoA apostilled by the concerned authority- district court in the Netherlands (Rechtbank). Embassy will attest the PoA for such categories of applicants only after those are apostilled.

In case of Attestation of Affidavit (civil matters only)
1. Original Affidavit Documents and copy of the same
2. Applicant to affix signature on each page of the document (before Embassy staff at the counter) – fee calculated as per signatures to be attested. No witnesses are required in this case.
